2014 Portland Steelheads Exemption Request for Blake Kingsley

Blake is a long time Steelhead who played in the Stumptown Throwdown (for Sacramento - but didn't get input into the USAFL system for some reason) and played in multiple PAFL games. He broke his wrist during the Throwdown and was unable to participate in our matches against Seattle and Sacramento. He was not planning on going to Nationals but has changed his mind and therefore also did not register and pay on time.
